The train, traveling at about 24 mph, crashed head-on with another train near Talerddig in Powys on October 21. The second train was cruising at a speed of around 6 mph at the time of the collision. Those critically injured included the guard of the faulty train and the driver of the other train.

The fact that the westbound train failed to stop in a designated loop to allow an eastbound train to pass on the single track didn't help much. The train's driver had applied the brakes as they approached the loop, but, due to the blocked hose and a few other issues, the wheels started to slide, resulting in an emergency brake demand.

Although the RAIB found that the sanding hoses were blocked with leaves and debris, a quick "safety check" carried out the day before suggested the system was not blocked at that time. The investigation is still ongoing to consider factors like sanding systems and the cause and severity of low adhesion at Talerddig.
